Approaching health with the future in mind

I am stuck in a negative thought pattern about my health. I have health circumstances, but know that a lot of how I feel is in my control emotionally and through my habits. As I have been watching my thoughts I see the negative thoughts and am trying to focus on being more forward thinking about what I want my health to look like in the future, specifically in one year:
• Off of all my medication
• Prioritize healthy eating
• Practice daily movement
• Weigh 135-140

Am I on the right track with this approach?
