Close but no cigar

I historically have come close to successes and then somehow either give them away, let them slip through my hands, etc. (“Close but no cigar”)
Could you take a look at this model please and let me know any feedback on it that would help me process this further?

T: I want to be that healthy self-caring person I know I’m capable of being, but I’m sure I won’t end up achieving it (in the end I’ll let it slip away)
F: unimportant
A: Not taking care of myself, sometimes mistreating myself (overeating), not prioritizing my future and my health
R: I’m getting heavier and unhealthier.
