Can you help with this instruction? It is supposed to be the model at the time of making the decision?

For example:

C where I live
T from 1994: I’m making a fresh start
F from 1994: Excited
A Moved to current city in 1994
R Started fresh in 1994

Or is it supposed to be a model in the present?

C where I live
current T: This might not be the right place to live
current F: uncertain
A continue live in current city
R: where I live doesn’t feel right
