What exactly is the difference between an Action and a Worthy Fail?

If I’m working on body image, is “walk by a mirror and not criticize myself” an Action or a Worthy Fail?

or

“Allow the feeling of being bloated and uncomfortable and don’t change my protocol,” is that an Action or a Worthy Fail?
